What is Blepharoplasty?

Early signs of the normal aging process include downward shifting of the brows, upper eyelid fullness, lower eyelid “bags” and puffiness, and wrinkling skin. Cosmetic eyelid surgery has gained popularity by giving the face a more youthful look by improving wrinkled, sagging eyelid skin. Correcting eye aging is a simple, effective way to achieve a rejuvenated yet natural look.

Initially, swelling of the upper eyelids presents only in the morning and diminishes by the end of the day. Allergies, systemic conditions, anatomical characteristics, or genetic factors may cause recurrent swelling that leads to earlier signs of aging in the upper eyelids. However, the swelling progressively worsens and does not resolve. Eventually, eye shadow smudges at the eyelid creases. Progressive upper eyelid fullness produces overhanging skin folds that result in partial visual obstruction.

Blepharoplasty, commonly known as eyelid surgery or cosmetic eyelid surgery, is a surgical procedure that improves the appearance of the upper eyelids, lower eyelids, or both. By removing excess skin, repositioning or removing excess fat, and refining the underlying muscle along with other tissues, the procedure creates smoother eyelid contours and a more alert appearance.

Upper lid blepharoplasty focuses on correcting drooping upper lids and excess eyelid skin that may even interfere with peripheral vision. What is the Blepharoplasty procedure like?

Your experience begins with a thorough consultation as you plan blepharoplasty surgery, where Dr. Jordan evaluates your eyelid anatomy, skin quality, medical history, vision concerns, and aesthetic goals. Together, you'll discuss whether upper blepharoplasty, lower blepharoplasty, or a combined approach is the best fit among available eyelid surgery procedures. It’s also wise to choose a qualified surgeon and verify board certification before moving forward.

These procedures may be performed under local, IV sedation, or general anesthesia. Dr. Trimas will discuss these options with you at your cosmetic consultation appointment. For upper eyelid surgery, incisions are carefully placed within the natural eyelid crease. For lower eyelid surgery, incisions may be hidden just beneath the lash line or inside the lower eyelid, depending on your anatomy and treatment plan.

Through these carefully concealed incisions, Dr. Jordan removes excess skin, may remove excess fat where needed, and refines the surrounding tissues to create smoother, more youthful contours. Once the desired result has been achieved, the incisions are meticulously closed to promote discreet healing.

Upper Blepharoplasty (Upper Eyelids)

Surgery Morning

  • Nothing to eat or drink after midnight-may brush teeth. (if undergoing anesthesia)
  • Take a shower and wash hair and face in the morning.
  • Do not wear any make-up (includes mascara and eyeliner). Wash face with baby shampoo when arrive at surgery center to remove any residual eye make-up.
  • Leave all jewelry at home. Cannot have any metal on your body, so please remove any body piercing.
  • DO NOT WEAR CONTACTS.
  • Clothing: Button-up shirt, sweat pants, socks, shoes (comfortable and easy to walk in).
  • Bring pain medication with you. You will be numb after surgery and may not feel anything. However, the nurses will give you a pain pill prior to discharge to help keep pain level at a minimum.

Upper Blepharoplasty (Upper Eyelids)

Surgery Night

  • Soft Bland Diet: DO NOT EAT SPICY FOODS and drink plenty of fluids. (if undergoing anesthesia)
  • Start your antibiotics at suppertime; IF NAUSEATED may start antibiotics the next day.
  • Sleep with head elevated using a recliner or at least 3-4 pillows for a week.
  • Apply ice compresses to eyes every hour for at least 15 minutes for a couple of days. Frequent application of ice compresses decreases swelling and bruising.
  • Take pain medication, as you need it, do not try to brave the pain. You will be more comfortable if you keep your pain at a minimum.

Upper Blepharoplasty (Upper Eyelids)

Next 2 weeks

  • Incision Care
  • Wash incision twice daily. Use Baby Shampoo or Cetaphil Soap. Clean incision sites by gently massaging with your fingers (DO NOT RUB HARD).
  • Do not face shower, a strong water pressure may open incisions.
  • Apply ANTIBIOTIC OINTMENT or AQUAPHOR to keep incisions moist throughout the day.
  • Expect EYE swelling, bruising, and redness for about 2 weeks (more or less).
  • Do not wear make-up until completely healed.
  • May wear contacts after 1-2 weeks.
  • Sutures:
  • Sutures-dissolvable, keep moist with ointment
  • Purple Glue-let it flake off (DO NOT PICK OFF)
  • No exercise, heavy lifting, or bending over.
  • Scars are to be expected, but hidden in the eye fold.

Surgery Morning

  • Use ARTIFICIAL TEARS frequently to rinse eyes of any drainage and keep eyes moist. Apply Bacitracin Ophthalmic Ointment inside eyes for 2 nights only.
  • Take pain medication, as you need it, do not try to brave the pain. You will be more comfortable if you keep your pain at a minimum.

Lower Blepharoplasty (Lower Eyelids)

Next 2 weeks

  • Skin Care– (if having lower eyelid CO2 Laser)
  • Every 2 hours: Wash with Cetaphil (Removing all of Aquaphor)
  • Wet gauze and gently scrub any yellow/white skin crusting
  • Pat Dry
  • Apply Aquaphor (may reapply if melts/dries off)
  • Some areas may bleed (just apply slight pressure) scrub around it and prevent scabbing by keeping moist with Aquaphor.
  • Take pain medication 30minutes-1 hour prior to washing.
  • Avoid sun exposure as much as possible.
  • Incision Care– (if having lower eyelid SKIN PINCH)
  • Clean with Cetaphil or Baby Shampoo. Apply antibiotic ointment or Aquaphor atleast twice daily. Sutures are dissolvable, keep moist with ointment. Let purple glue flake off.
  • Expect EYE swelling, bruising, irritation, and redness for about 2 weeks (more or less).
  • Use Artificial Tears to keep eyes moist. Use Blephamide drops if experience irritation and redness.
  • No exercise, heavy lifting, or bending over.
  • DO NOT WEAR CONTACTS
  • Do not scratch lasered areas, may cause scarring.

Lower Blepharoplasty (Lower Eyelids)

After 2 weeks (if having lower eyelid CO2 Laser)

  • Continue Facial Care: (start when skin is pink)
  • Wash with Cetaphil morning and night
  • Pat Dry
  • Apply Cetaphil moisturizer with SPF 30 throughout the day
  • Sun protection is very important. Wear a hat, sunglasses, and sunscreen (at least SPF30) at all times.
  • May wear make-up.

Am I a candidate for Blepharoplasty?

Lower eyelid puffiness or lower eyelid “bags” is caused by the protrusion of fatty tissues under the lids. It is sometimes noticeable at a relatively young age, beginning in the mid-twenties. More often, it is developmental in nature and appears at thirty-five to forty years old, becoming more noticeable in the forties or fifties. Depending on your needs, Dr. Jordan will either perform a “skin pinch” to remove excess skin and sagging on the lower eyelids or laser resurfacing to improve fine lines, discoloration, or “dark circles” beneath the eyes. If you are experiencing one or more of these signs, you may be a good candidate for an Upper and/or Lower Blepharoplasty procedure.

Ideal candidates are generally healthy and should avoid tobacco for four to six weeks before surgery, with realistic expectations. During your consultation, Dr. Jordan will review your medical history, medications including blood thinners and other blood thinners, thyroid conditions, dry eye symptoms, previous surgeries, and aesthetic goals before recommending the safest treatment plan for you.

Swelling and bruising are common after surgery and typically last 10 to 14 days, with most patients noticing steady improvement each day. Cold compresses, keeping your head elevated, and limiting strenuous activity can help support healing and minimize swelling, especially during the first 2 to 3 days post-surgery. Stitches in the upper eyelids typically remain for about one week.

Some patients experience temporary light sensitivity, dry eyes, or slight discomfort during the early recovery period. Most people feel comfortable returning to work and social activities within about two weeks, although subtle healing continues over the following several weeks. Dr. Jordan and the Couture Plastic Surgery team remain available throughout your recovery to answer questions and monitor your progress. You may also receive different instructions from your surgeon based on your procedure and healing progress.
